Payment is due when services are rendered on all self-paying or insurance patients (deductibles or your percent). If we do not accept your insurance, you must pay up front and a claim will be provided for you to mail to your insurance carrier.
Insurance balance must be paid before future appointments can be made.
If your account has a credit balance of less than $25.00 it will be your responsibility to contact our office for your refund.

Patients have a certain time slot reserved for their visit. If you're unable to keep that time, a 48-hour notice is required. Otherwise, there will be a $50.00 charge in order to re-schedule your appointment. After two broken appointments without a 48-hour notice, future appointments will not be made. We work hard to stay on schedule, so if you are 15 minutes late or more, there is a chance we will need to re-schedule.
If you think you might be late, please call and notify the receptionist.
The legal guardian must accompany patients on the first visit and, if possible, on future visits. If the legal guardian is a grandparent, aunt, uncle, etc., then legal documentation must be brought on the first visit before any treatment begins.
Only the person(s) specified by the legal guardian on the HIPPA form provided may bring the patient(s) to future visits, make appointments, and have dental information released to them.
Parents are allowed in the back with the patients only for cleanings, X-rays, and exams, but not when treatment (fillings, extractions, crowns, etc.) is being performed.
Patients who have insurance must have their insurance card or insurance information (group#, mailing address, phone #, and the insured's information) on the initial visit or on future visits when the insurance has changed.

Dental insurance is not meant to be a pay-all, but rather a pay aid. Your plan might tell you that you will be covered up to 80-100%. In spite of that, most plans are found to cover only 15-50% of an average fee. The amount your plan covers is determined by how much your employer pays for the plan. The less paid for the insurance, the less you will receive.
If you have any questions regarding your insurance, you should contact your company regarding the details of your plan. We are happy to submit your claims, and we will try to get the maximum benefit that your plan provides. It is the patient/subscriber's responsibility to know their insurance plan and coverage.
